According to a report in the American Journal of Health-System Pharmacy, nearly 20 percent of patients suffer adverse events shortly after being discharged from the hospital. Often, such adverse events are preventable. To reduce the number of these events and the readmissions that often result, the patient and/or caregiver must clearly understand and comply with the original hospital discharge instructions, putting much emphasis on the notion of self-care.
